Pediatric Orthopaedic Conditions and Treatments at SV Ortho Care
Cerebral Palsy (CP)
Overview:
Cerebral palsy is caused by irreversible brain damage occurring before, during, or shortly after birth. This damage leads to muscle tightness (spasticity) primarily in the limbs and sometimes the trunk, resulting in limited movement.
Treatment Approaches:
Physiotherapy: To improve muscle flexibility and strength.
Medications: Botox injections to reduce muscle tightness.
Surgical Interventions:
Correction of muscle contractures
Tendon transfer surgeries to balance muscle forces
Bony surgeries such as pelvic osteotomies to stabilize the hip and foot correction procedures
Club Foot (Congenital Talipes Equinovarus - CTEV)
Overview:
Club foot is the most common congenital deformity requiring treatment, where the foot is twisted inward and downward.
Treatment:
Ponseti Technique: Serial plaster casting to gradually correct the deformity, now the gold standard.
Surgery: For neglected or severe cases, open surgery or external fixation devices may be required.
Developmental Dysplasia of the Hip (DDH)
Overview:
Previously called congenital dislocation of the hip, DDH involves improper formation or dislocation of the hip joint.
Treatment:
Early Diagnosis: Managed with bracing to guide proper hip development.
Surgery: In delayed or failed cases, reconstructive pelvic osteotomies are performed to rebuild and stabilize the hip socket.
Limb Deformity Correction
Overview:
Shortening or deformity of limbs can result from congenital issues, infections, trauma, or other causes.
Treatment:
Use of specialized external fixation devices called fixators that gradually correct deformities and lengthen bones.
Congenital Pseudarthrosis of Tibia
Overview:
A rare condition where the tibia fails to form a solid bone, resulting in a false joint.
Treatment:
Surgical intervention is essential to promote healing and restore limb function.
Muscular Dystrophy
Overview:
A group of hereditary diseases causing progressive muscle weakness and degeneration. Common types include Duchenne and Becker muscular dystrophies.
Treatment:
Surgical correction for limb or spine deformities and hip joint stabilization are often necessary to improve mobility and quality of life.
Obstetric Brachial Plexus Palsy (OBPP)
Overview:
A birth injury affecting nerves supplying the shoulder and arm, with an incidence of 1.5 per 1000 births. Risk is increased in cases of shoulder dystocia and large babies but reduced with cesarean delivery or multiple births.
Treatment:
Reconstructive surgeries including contracture release
Corrective bone osteotomies
Modified quadrilateral (quads) procedures to restore function
